How much does it cost to be a member of Mountain Sage Medical Direct?
After your enrollment – you will be charged $39 per member per month the first of each month and a $20.00 copay fee at each office visit (Medicare patients do not have a $20 copay). We do not bill your insurance. There are no hidden fees. It really is that simple. We do not discriminate based on age, past medical history, or financial background. If we do a covered procedure or a standard test in the office (such as an injections, labs, or sutures) then it is at no additional cost to you. When we order a laboratory study that is not part of our standard panel you will charged our cost plus $12 for processing and the lab draw. Why would I pay "extra" to be a member of Mountain Sage Medical Direct?
1) For any patients that historically do not reach their deductible – DPC should be LESS expensive than using the third party system for primary care.
2) You will see the same Dr. Fitzpatrick at each visit and if you prefer our office arrange E-visits (using your computer) to address some issues if you are out of town.
3) Our shortest office visit is 30 minutes and commonly up to an hour,so your problems can be addressed more effectively.When you make your appointment we will allot the time needed to address your concerns.
For Medicare patients, the decision is more difficult for both you as a patient and myself as your physician. I would prefer not to stop taking Medicare, but the cost to comply with the many regulations and progressively less reimbursement makes very difficult to stay viable in a small practice setting.
We completely understand that the $39.00 fee may be beyond your budget for some of you,but we would ask that if you would like to stay with us and cannot afford the fee to let us know, as we will work with you to try and find a solution.
Why do you not bill/accept traditional insurance, Medicare, or Medicaid?
We felt doing this would defeat the purpose of Direct Primary Care. Your medical care would be far more expensive due to the overhead associated with these third party claims, and you would no longer be in control of medical decision making. In fact, we take the fully committed step of “opting out” of Medicare so that we are permitted to privately contract with Medicare patients in this model. We felt is was important to keep our practice open to any and all patients, regardless of insurance status. We do not start our phone conversations with “what is your insurance.” Our fees will be considered “out of network,” and depending upon your insurance plan may not apply toward your deductible.
Am I stuck in a long term contract? What if I do not like the practice after I join?
No, you are never under any obligation to continue your membership and may discontinue it at any time without a penalty. Please do keep in mind that if you later wanted to restart your membership you would need to pay a re enrollment fee of 100.00 to cover administrative costs that is not charged on initial enrollment.If the physician patient panels are full, then you would be unable to rejoin the practice until a position opened. If for some reason you are unsatisfied with your level of care, please let us know.
May I schedule an appointment without becoming a member?
Yes: Mountain Sage Medical Direct has a CASH OPTION that anyone can take advantage of;
The cost for a nonmember is $69.00 for a physician visit only. If additional procedure or labs are necessary there is a $39.00 fee to cover these costs.Members have priority and a nonmember can be seen if there is an opening.
What are your usual hours?
Monday-Thursday, 9 am-4 pm, and Friday until noon; If necessary, Dr. Fitzpatrick may schedule home visits. You may contact Mountain Sage Medical Direct via phone or email and we will respond usually within 24hr, or anytime during business hours by using MYUPDOx (please call to ask us how to obtain a sign-in so you can use MYUPDOX)
May I simply walk in?
You may do this during our regular hours , but we do encourage you to notify us in advance so that you may avoid wait times as scheduled patients will be the priority. Contacting us in advance allows us to determine when it is best for you to arrive so that minimal wait is required (the norm for our patients) and will assure that you do not attempt to be seen on a day when Dr. Fitzpatrick happens to be out of town.
Are the services offered by Mountain Sage Medical Direct considered Insurance?
No. DPC is NOT insurance. We recommend (but do not require) that all patients have some form of health insurance. We will do our best to help you best utilize your insurance when it makes sense to do so. If the cash pay price of a medication/procedure is less than using your insurance, then we will do our best to advise you in this regard. If you do not have (and cannot afford) traditional health insurance, we might suggest that you consider joining one of the health sharing ministry programs which are easily searched on the internet such as:

Will you do house calls?
Yes, if you live in Burns/Hines we are happy to provide this service with patients who have special needs.
Why do you call it "Direct Primary Care?" Is this not re-labeled "Concierge Medicine?"
No, DPC does not “double dip.” Most concierge (i.e. fee for non-covered services) practices bill traditional insurance, Medicare, or Medicaid in same old inefficient fee for service fashion and then charge the patient a monthly fee in addition to those other monthly charges.
What services are offered at Mountain Sage Medical Direct?
Please see our service offerings page, but basically our traditional office practice will continue with some added benefits for patients in terms of labs, imaging, etc
What services are not offered at Mountain Sage Medical Direct?
We do NOT routinely prescribe narcotics for chronic pain, benzodiazepines for anxiety, or stimulants for ADHD. We also do not prescribe birth control as we prefer this is managed by the patient’s’ Obstetrician/Gynecologist.We do not stock vaccinations at this time because these can be obtained in a more cost-effective manner outside of our office, and will be happy to direct you to one of our local pharmacies (Safeway or Rite-Aid) or the health office for the needed vaccinations.
